ACL Surgery
When is it necessary to combine lateral extra-articular tenodesis (LET) or ALL reconstruction with ACL reconstruction?
Author: Dr. Jiwu Chen
Doctor’s answer
In cases of ACL revision, severe rotational instability (pivot shift ++/+++), generalized ligamentous laxity, knee hyperextension, or high-intensity pivoting athletes, the risk of re-rupture after isolated intra-articular ACL reconstruction is relatively high. Adding anterolateral ligament (ALL) reconstruction or lateral extra-articular tenodesis (LET/Modified Lemaire) can effectively enhance the knee's resistance to rotational shear forces, reducing the rate of graft re-tear.
