Revision ACL surgery
When is two-stage revision ACL reconstruction with staged bone grafting considered?
Author: Dr. Jiwu Chen
Doctor’s answer
The decision for single-stage versus two-stage revision ACL reconstruction depends on previous tunnel enlargement, tunnel malposition, and potential geometric conflict with planned anatomic tunnels. If severe osteolysis or convergence prevents secure graft fixation and aperture support, a staged approach (Stage 1 hardware removal and dense bone grafting; Stage 2 anatomic reconstruction after bony consolidation) is recommended; non-conflicting tunnels may proceed in a single stage.
