Complex knee ligament injuries
How are posterolateral corner (PLC) knee injuries assessed, and when is surgical intervention needed?
Author: Dr. Jiwu Chen
Doctor’s answer
The posterolateral corner (PLC) comprises the fibular collateral ligament, popliteus tendon, and popliteofibular ligament. The Dial test assesses external rotation asymmetry at 30° and 90° of knee flexion, differentiating isolated PLC injury from combined PLC-PCL disruptions. Mild isolated injuries (Grades I-II) are often managed non-operatively with bracing, whereas severe Grade III tears or multiligament combined injuries typically warrant anatomic repair or reconstruction.
